Acute Lymphoblastic Leukaemia (ALL)

Acute Lymphoblastic Leukaemia (ALL), also known as Acute Lymphocytic Leukaemia, is a type of blood cancer that affects the white blood cells and the bone marrow. Bone marrow is the soft tissue inside bones where blood cells are produced.

It is a rapidly progressing cancer, meaning the abnormal cells multiply very quickly and interfere with the normal production of healthy blood cells. ALL is considered the most common type of cancer in children, although it can also affect adults.

Recent Study on TP53 Mutation

A recent multi-institutional study involving 830 adult ALL patients across eight academic centres found that nearly one in every ten adult patients diagnosed with ALL carried a mutation in the TP53 gene.

The TP53 gene plays a crucial role in controlling cell growth and preventing tumour formation. Mutations in this gene are often associated with aggressive cancers, poor treatment response, and lower survival rates. This finding is important because it may help doctors develop more targeted and personalized treatment strategies for ALL patients.

Causes of Acute Lymphoblastic Leukaemia

ALL develops when a cell in the bone marrow undergoes genetic mutations or changes in its DNA. These mutations cause immature white blood cells, called lymphoblasts, to grow uncontrollably instead of developing into healthy blood cells.

As these abnormal cells accumulate, they crowd out normal blood cells, leading to infections, anaemia, and bleeding problems.

Symptoms of ALL

The disease can produce several symptoms due to the reduced production of healthy blood cells. Common symptoms include:

  • Bleeding from gums

  • Bone pain

  • Fever

  • Frequent infections

  • Frequent or severe nosebleeds

  • Weakness and fatigue

  • Swollen lymph nodes

Because ALL progresses rapidly, early diagnosis and treatment are extremely important.

Types of Acute Lymphoblastic Leukaemia

According to the World Health Organization (WHO), ALL is broadly classified into two major categories:

1. B-cell Lymphoblastic Leukaemia/Lymphoma

This type begins in immature cells that normally develop into B-lymphocytes, which help the body fight infections by producing antibodies.

It is the most common subtype of ALL.

2. T-cell Lymphoblastic Leukaemia

This type starts in immature cells that normally develop into T-lymphocytes, which are involved in immune defence and destroying infected cells.

It is comparatively less common than B-cell ALL.

Treatment of ALL

Treatment for ALL usually depends on the patient’s age, subtype of cancer, and genetic mutations present in the cells. Common treatment methods include:

Chemotherapy

It uses powerful drugs to destroy rapidly growing cancer cells and is the most widely used treatment for ALL.

Targeted Therapy

Doctors may use targeted drugs that specifically attack cancer cells with particular genetic mutations, such as TP53-related abnormalities.

Other Treatments

In some cases, patients may also require:

  • Immunotherapy

  • Radiation therapy

  • Bone marrow or stem cell transplantation

Acute Lymphoblastic Leukaemia is a serious but treatable blood cancer that mainly affects the bone marrow and white blood cells. The recent discovery of TP53 mutations in a significant number of adult ALL patients highlights the growing importance of genetic research in cancer diagnosis and treatment. Advancements in targeted therapies and early detection can improve survival outcomes and help develop more personalized treatment approaches.
